Docnote
Docnote is a secure digital platform designed to streamline the management of uninsured services.
Our office will begin using Docnote when charging for uninsured services. Docnote is intended specifically for requesting uninsured services. It should not be used to schedule appointments or ask general questions.
What are uninsured services?
Uninsured services are medical services not covered by the Ontario Health Insurance Plan (OHIP). These typically include:

Sick notes, Attending Physician Statements, and other forms

Completion of insurance, disability, or work-related documentation

Prescription renewals
-
Travel immunizations
-
Medical procedures and reports
Setting Up Your Docnote Account
To create your secure Docnote account, visit https://www.docnote.com/.

Under "For Patients" select "Enter Here".

On the top right corner select "Login/Sign up" and search your doctor's name.

Complete the required patient information fields (this includes your first and last name, your e-mail, your date of birth, your Canadian mobile number and Health Card Number) and create a password. Review and agree to Docnote’s Terms and Conditions

You will receive a one-time, two-step verification code to your email and mobile number.

Enter both verification codes to complete your account setup.
Once your Docnote account is created, you’ll be able to easily submit requests, make payments, and manage your uninsured services—securely and conveniently.
If you have any questions or need assistance setting up your account, please contact Docnote directly at 1-866-236-6172 or support@docnote.com.
To help maintain office efficiency and keep phone lines available, please do not contact the office regarding any issues with your Docnote account. This will help reduce unnecessary calls and ensure better access for all patients.

Blood work services - Update
Changes to our Blood Test Service
For many years, we have offered blood tests right here at the clinic. We know this service is very helpful. Sadly, we now need to make some changes.
👉 Why?

OHIP only pays for a very small part of the costs.

Most of the costs are paid by the doctors.
-
With prices going up every year, it is no longer possible to keep offering the service the same way.
👉 Our situation

We are one of very few clinics in the region that still offers this service.

The law does not allow us to charge patients for blood tests.
-
If we do not make changes, we may have to stop this service completely — something we want to avoid.
👉 What this means for you

Blood tests at the clinic are now available only at certain times and for certain patients.

We understand this is disappointing.

Other options are available: several labs and hospitals in the area continue to offer blood tests.

We would love to continue “as before,” but the doctors can no longer cover all the costs. We thank you for your patience and support as we do our best to keep this service available.

Participation in a research study
Doctors at St Isidore Medical have chosen to participate in a study to evaluate the Aline-Chrétien Health Hub. To make this research feasible, the Canadian Primary Care Information Network will be used in the practice.
Participation in the study is completely voluntary, and your decision whether or not to participate will not affect your care in any way. Please contact the research team at evaluationcso@montfort.on.ca or see this practice’s front office staff for more details.

Open House
On September 12 2024, we opened our doors to our patients, friends, families and collaborators. Welcoming everyone in our new facilities was such a pleasure for our team! It was a great night in good company!
